Decisions: propose, accept, supersede
Accepted decisions bind the records and work they govern. Proposed ones do not.

Steps
- Ask the agent to propose a decision when a design choice is made, or write one yourself.
- Link it to what it governs.
- Accept it; from then on agents treat it as binding.
- When it changes, accept a new decision that supersedes it.
States
Proposed → Accepted / Rejected → Superseded.
